Dear Friend,

Thank you for your request for a Cayce-based health evaluation. After you download and print out the form simply fill in your personal health history and complete the Cayce Comprehensive Symptom Inventory section.

Return the completed forms and a check made payable to the A.R.E. The charge is $60.00 for A.R.E. members and $ 70.00 for non-members. We are able, usually, to send the results in two business weeks from the date we receive your information.
